the 2 minute signup is a required online form that clients must complete immediately after signing their quote , which is sent to them by their broker this form collects essential information about the clientβs business operations, injury history, and payment preferences it plays a critical role in preparing the client for onboarding with our service partners π purpose of the form the 2 minute signup allows clients to confirm and update core business details provide company background and risk indicators submit previous injury records (if applicable) share underwriting related details about the business submit initial payment method and authorize fees π page by page breakdown β
page 1 β business information this page displays pre filled information based on what we already have on the client's lead card in januscrm clients are asked to review and update any fields if they are inaccurate or outdated fields typically include business name business address fein / tax id phone number email address website (optional) estimated yearly sales years in business payroll frequency and payroll start date fields not already filled in will be required before proceeding β οΈ page 2 β injury history this section captures whether the business has had any work related injuries involving employees if yes , clients are required to enter employee name nature of injury cost of injury insurance carrier at the time of incident ποΈ page 3 β company history (underwriting questionnaire) this page helps the underwriting team assess the type of work the client performs most of these questions are yes/no and relate to job site exposures subcontractor usage out of state operations use of ladders or scaffolding company ownership details π³ page 4 β payment method (one time payment form) clients are prompted to submit their preferred payment card for the initial cost fee this payment form is securely processed and used for a one time charge only once submitted the card is stored securely the form is routed to the billing profile under the correct purpose a confirmation email is sent to the client when any one time payment fails π³ new feature improved recovery process for declined one time payments what happens when a payment is declined user sees a declined message with clear next steps system automatically displays the decline reason provides immediate retry option guides user through resolution to recover from a declined payment review the decline message select update payment method either choose a different existing card add a new payment method resubmit immediately no need to restart the process β
success the payment can be resubmitted immediately after updating the payment method the original request remains active ποΈ final step β signature & confirmation after completing all required fields clients will sign the form electronically a thank you page confirms completion a copy of the completed form is emailed to the client the internal onboarding and underwriting teams the assigned broker this ensures visibility and helps move the client to the next phase of onboarding π οΈ internal routing & notifications once submitted all answers and attachments are saved to the lead card in januscrm internal teams (broker, underwriting, billing, enrollment) are automatically notified if injury history is marked yes , a copy of the form is also sent to the claims department
