π overview the worksite employee acknowledgment form allows users to send a digital acknowledgment to employees confirming receipt of employer provided documents this form can be sent to individual employees or all active employees associated with a client, directly from januscrm β οΈ important this feature is available only for compeo and peopaygo employees π where to find the form the worksite employee acknowledgment can be sent from the following locations within januscrm lead card enrollment tab employee subtab to access the form navigate to one of the locations above select send worksite forms ack π§Ύ how to send a worksite employee acknowledgment step 1 open the modal click send worksite forms ack a modal window will appear step 2 add forms for acknowledgment in the free text field, type the name of a document the employee needs to acknowledge receiving press enter on your keyboard or click the + button to add the document repeat this process for each document that applies you may add as many document names as needed document names should match what was provided to the employee step 3 choose recipients by default, the form will be sent to the selected employee to send the form to all active employees for the client check send to all active employees at the bottom of the modal the system will display the total number of employees who will receive the form step 4 send the form once at least one document is added, the send button becomes available click send to deliver the acknowledgment form β
confirmation if the form is sent successfully, a green confirmation banner will appear on the right side of the screen π§ what happens after the form is sent employee experience the employee receives an email with a secure link to the acknowledgment form the employee reviews the listed documents and signs electronically notifications after completion once the employee completes the form employee employer payroll advisor all receive an email with the signed acknowledgment form attached β³ expiration & unsigned forms if the employee does not sign within the required timeframe (same behavior as ls55 and ls57) the link expires all recipients receive a copy of the form marked with a βnot signedβ banner the form (signed or unsigned) is saved on the lead card for recordkeeping π examples of documents appropriate for this acknowledgment the worksite employee acknowledgment form is intended to confirm receipt of required notices, policies, or informational documents examples include, but are not limited to new health insurance marketplace coverage options or your health coverage blood donation leave notice pay notice and acknowledgement for farm workers (if applicable) notice of fringe benefits and hours model airborne infectious disease exposure prevention plan policy on the rights of employees to express breast milk in the workplace notice of electronic monitoring of employee communications paid leave policies sick leave paid time off (pto) other employer provided leave policies model language for employee materials sexual harassment policy for all employers in new york state employee handbook employee rights under the family and medical leave act (fmla) document names entered should reflect the exact titles used by the employer β οΈ important usage warning this form should only be sent when explicitly instructed by management or relevant stakeholders do not send this form proactively do not use this form without direction from management compliance legal authorized stakeholders this form is intended for acknowledgment and documentation only , not policy guidance or distribution decisions unauthorized or improper use may result in compliance or client risk π notes & limitations available only for compeo and peopaygo employees document entry is currently free text expiration, notification, and storage behavior mirrors ls55 and ls57
